Marami po ang nagtatanong sa amin kung papaano makakakuha ng PWD ID. Ito po ang unang certification na kailangan, ang Certification on Disability (i-click lang po para i download). Bukod pa po sa certification na ito, kalangan ninyo ng kahit alin sa mga sumusunod:
- Voter’s certificate
- Barangay certificate
- voter’s ID
Ginawa po namin ang update na ito upang malaman ng marami na mas mabilis na paraan ito upang makakuha ng PWD ID kaysa sa magdala kayo ng clinical abstract kung saan minsan mas nagtatagal ang pagkuha ng PWD ID.
I-download lang ang form at papirmahan sa inyong doktor bago isumite sa PDAO.

Just to clarify lang po, as per a personnel working in the PWD application section of Quezon City, chronic illnesses are no longer valid for PWD application. Is this true?

Yes, they didnt acknowledge that chronic illnesses are no longer accepted for PWD id application.

Pede po pa bang makapag apply ng PWD ung anak ko po na nadiagnose ng may mild ADHD last 2017? Wala pa po akong updated na evaluation po sa kanya ulit gawa ng sa August pa po ang schedule namin at 2018 na kami nakapag pa theraphy gawa ng financial reason at single mom din po ako. As of now , naghohomeschool din po kami. Recently ko lang po namalaman na pede ko siyang i apply para makatulong sa operation nya this coming June 13 gawa ng aayusin ung daanan ng ihi nya para di na lumala ang Chronic Kidney Disease stage 2 nya. Enough na po ba ung initial assessment nya po then letter from his Educational Theraphist na nag uundergo po sya ng theraphy?

Should not the PWD ID in cancer have expiration?

“Cancer patients, persons living with cancer, and cancer survivors are considered as persons with disabilities (PWDs) in accordance with RA 7277, the ‘Magna Carta for disabled persons.'” –ncda.gov.ph
Please notice that even “cancer survivors” are still considered PWDs implying that cancer is a permanent disability.
